Christmas Play Parent Form "*" indicates required fields Dear Parents, We are excited to be performing our annual Christmas Play at the Five Oaks Christmas Party at the Five Oaks park Saturday, December 13th. If you would like any of your kids to be in this play, please sign the form below and submit it or return it by October 30th. In order for your child/children to be in the play or dance, they must meet the following requirements: At least 4 years old Willing to participate in the practice and performance and follow directions Attend all practices noted below Arrive early the day of the play for an extra practice if needed Rehearsal times: Thursday October 30th Thursday November 6th Thursday November 13th Thursday December 4th (Dress Rehearsal) Thursday December 11th (Final Dress Rehearsal) I permit my child/children*to participate in the Christmas play and/or dance by having a part or helping with the younger kids backstage.
